If you want us to authorize and pay overdrafts on ATM and everyday debit card transactions, click on the "I Accept" button below and fill out the authorization form.
You can revoke your authorization for Collins Community Credit Union to pay these overdrafts by calling (319) 393-9000 option 3, coming by a branch, by visiting our website at https://www.collinscu.org and clicking decline on the Online Opt In Form, or by mailing a revocation of consent to 1005 Blairs Ferry Road NE, Cedar Rapids, IA 52402. Your revocation must include both your name and your account number so we can properly identify your account.
